Towards Amyotrophic Lateral Sclerosis Interpretable Diagnosis Using Surface Electromyography
Amyotrophic Lateral Sclerosis (ALS) is a fast-progressing disease with no cure. It is
diagnosed through the assessment of clinical exams, such as needle electromyography,
which measures themuscles’ electrical activity by inserting a needle into themuscle tissue.
Nevertheless, surface electromyography (SEMG) is emerging as a more practical and less
painful alternative. Even though these exams provide relevant information regarding the
electric structures conducted in the muscles, ALS symptoms are similar to those of other
neurological disorders, preventing a faster detection of the disease.
This dissertation focuses on implementing and analyzing innovative SEMG features
related to the morphology of the functional structures present in the signal. To assess the
efficiency of these features, a framework is proposed, aiming to distinguish healthy from
pathological signals through the use of a classification algorithm. The classification task
was performed using SEMG signals acquired from the upper limb muscles of healthy and
ALS subjects.
The results show the utility of employing the proposed set of features for ALS diagnosis,
with an F1 Score higher than 80% in most experimental conditions. The novel features
improved the model’s overall performance when combined with other state-of-art SEMG
features and also demonstrated efficiency when used individually. These outcomes are
of significant importance in supporting the use of SEMG as a complementary diagnosis
exam. The proposed features demonstrate promising contributions for better and faster
detection of ALS and increased classification interpretabilityA Esclerose Lateral Amiotrófica (ELA) é uma doença incurável de progressão rápida. O
Geothermal resources research in a granitic basement - the Braga area case study (NW Portugal)
Geothermal resources are increasingly being considered as a strategic alternative in energy production, especially with the latest geopolitical developments. The densely populated Braga region, in NW Portugal, is endowed with a geostructural setting that enables the existence of several thermal water occurrences, spatially associated with a deep-rooted structure – the Vigo-Régua shear zone, set in a granite context. Given the latest advances in geothermal energy production, it is possible to predict a mid- to long-term implementation of geothermal energy production in the vicinity of that deep rooted structure. Although strongly encouraging, the exploratory geophysical, geochemical and geological data are still insufficient to deliver a definitive frame of the potential energy associated with the estimated reservoirs. Ongoing work combining gravimetric, radiometric and geochemical data will provide a better understanding of the deeply concealed structures.
Prevalence, associated factors and outcomes of pressure injuries in adult intensive care unit patients: the DecubICUs study
Funder: European Society of Intensive Care Medicine; doi: http://dx.doi.org/10.13039/501100013347Funder: Flemish Society for Critical Care NursesAbstract: Purpose: Intensive care unit (ICU) patients are particularly susceptible to developing pressure injuries. Epidemiologic data is however unavailable. We aimed to provide an international picture of the extent of pressure injuries and factors associated with ICU-acquired pressure injuries in adult ICU patients. Methods: International 1-day point-prevalence study; follow-up for outcome assessment until hospital discharge (maximum 12 weeks). Factors associated with ICU-acquired pressure injury and hospital mortality were assessed by generalised linear mixed-effects regression analysis. Results: Data from 13,254 patients in 1117 ICUs (90 countries) revealed 6747 pressure injuries; 3997 (59.2%) were ICU-acquired. Overall prevalence was 26.6% (95% confidence interval [CI] 25.9–27.3). ICU-acquired prevalence was 16.2% (95% CI 15.6–16.8). Sacrum (37%) and heels (19.5%) were most affected. Factors independently associated with ICU-acquired pressure injuries were older age, male sex, being underweight, emergency surgery, higher Simplified Acute Physiology Score II, Braden score 3 days, comorbidities (chronic obstructive pulmonary disease, immunodeficiency), organ support (renal replacement, mechanical ventilation on ICU admission), and being in a low or lower-middle income-economy. Gradually increasing associations with mortality were identified for increasing severity of pressure injury: stage I (odds ratio [OR] 1.5; 95% CI 1.2–1.8), stage II (OR 1.6; 95% CI 1.4–1.9), and stage III or worse (OR 2.8; 95% CI 2.3–3.3). Conclusion: Pressure injuries are common in adult ICU patients. ICU-acquired pressure injuries are associated with mainly intrinsic factors and mortality. Optimal care standards, increased awareness, appropriate resource allocation, and further research into optimal prevention are pivotal to tackle this important patient safety threat

disciplinar leccionada e a operatividade profissional de software multimédia educativo;
It can be understood from literature on Learning Technology that authors seek to
define the attributes necessary for educational software with reference to a paradigm
which justifies the use of technology as a cognitive learning strategy.
Taking into account the subject being studied, this research is intended to provide
generic evaluation criteria which will assist in decision-making with regard to the
operability of educational software by relating them with the acquisition of learning. We
have, to this end, established the following objectives: 1) To develop an instrument for
the evaluation of educational multimedia software; 2) To compare the subject area
taught and the professional operability of educational multimedia software; 3) To
understand how this operability in turn influences the evaluation of pedagogical,
ftanctional and technical criteria being studied; 4) To discover to what extent the
professional operability of software can constitute support for learning.
The referentialisation process performed on the subject under analysis entailed
applying a questionnaire in an exploratory study developed using research
methodologies of a quantitative-correlating and qualitative nature.
We concluded that: a) The criteria for evaluating the quality of educational
multimedia software delimit themselves and delimit their pedagogical, functional and
technical dimensions, with teachers placing more value on pedagogical attributes, the
importance of curricular contents and specific technical and ftanctional aspects; b)
Teachers in different fields / subjects do not assign the same importance to the
professional operability of educational multimedia software; c) For professional
operability, to wit, the type of software which is the most used, teachers state
significantly different quality criteria, so that the software may better fulfil the needs of
its users ; d) Learning seems to be influenced by the use (or otherwise) of software in the
classroom and emphasis placed on multimedia learning options.

Unraveling the genetic background of individuals with a clinical familial hypercholesterolemia phenotype
Familial hypercholesterolemia (FH) is a common genetic disorder of lipid metabolism caused by pathogenic/likely pathogenic variants in LDLR, APOB, and PCSK9 genes. Variants in FH-phenocopy genes (LDLRAP1, APOE, LIPA, ABCG5, and ABCG8), polygenic hypercholesterolemia, and hyperlipoprotein (a) [Lp(a)] can also mimic a clinical FH phenotype. We aim to present a new diagnostic tool to unravel the genetic background of clinical FH phenotype. Biochemical and genetic study was performed in 1,005 individuals with clinical diagnosis of FH, referred to the Portuguese FH Study. A next-generation sequencing panel, covering eight genes and eight SNPs to determine LDL-C polygenic risk score and LPA genetic score, was validated, and used in this study. FH was genetically confirmed in 417 index cases: 408 heterozygotes and 9 homozygotes. Cascade screening increased the identification to 1,000 FH individuals, including 11 homozygotes. FH-negative individuals (phenotype positive and genotype negative) have Lp(a) >50 mg/dl (30%), high polygenic risk score (16%), other monogenic lipid metabolism disorders (1%), and heterozygous pathogenic variants in FH-phenocopy genes (2%). Heterozygous variants of uncertain significance were identified in primary genes (12%) and phenocopy genes (7%). Overall, 42% of our cohort was genetically confirmed with FH. In the remaining individuals, other causes for high LDL-C were identified in 68%. Hyper-Lp(a) or polygenic hypercholesterolemia may be the cause of the clinical FH phenotype in almost half of FH-negative individuals. A small part has pathogenic variants in ABCG5/ABCG8 in heterozygosity that can cause hypercholesterolemia and should be further investigated. This extended next-generation sequencing panel identifies individuals with FH and FH-phenocopies, allowing to personalize each person’s treatment according to the affected pathway
